Many parents assume orthodontics starts in middle school, once all the permanent teeth are in. In reality, an orthodontist can learn a great deal from a much younger child’s mouth. A first checkup at the right time doesn’t mean your child is headed for braces. It simply gives you a clear picture of how their teeth and jaws are developing, while there’s still plenty of growth ahead.
The Age 7 Recommendation
A child’s first orthodontic checkup should happen when a problem is first noticed, and no later than age 7, according to the American Association of Orthodontists. That doesn’t mean something is wrong at 7. It’s a sensible point to take a look, even when everything seems fine at home.

Why This Age Makes Sense
By around age 7, most children have a mix of baby teeth and permanent teeth, and their first permanent molars have usually come in. Those molars show how the back teeth meet, which tells the orthodontist a lot about how the bite is developing. The jaws are also still growing, so any concerns can be tracked over time and addressed during a window when growth can work in your child’s favor.
What an Early Evaluation Looks For
The visit is quick and comfortable for most kids. The orthodontist checks how the upper and lower teeth line up from front to back and side to side, and looks at whether there’s enough room for the permanent teeth that haven’t arrived yet. Other things they pay attention to include:
- Crossbites, where some upper teeth sit inside the lower teeth
- Front teeth that stick out or don’t meet when the back teeth close
- Crowding or unusually large spaces
- Baby teeth lost much earlier or later than expected
- Habits such as thumb sucking or frequent mouth breathing
- Signs that the jaw is growing unevenly

An Evaluation Doesn’t Always Mean Treatment
This is the part that surprises most parents. An early checkup usually ends one of three ways: no treatment is expected, treatment may be needed later so growth is monitored, or a developing problem is found that suits early treatment. Most children fall into the first two groups.
When early care is recommended, it’s typically a limited first phase aimed at one specific issue. A palatal expander, for example, can widen a narrow upper jaw, and the AAO notes that palatal expansion is ideally done while a child is still growing. What If Your Child Is Younger or Older Than 7?
Age 7 is a ceiling, not a starting line. If you notice a crossbite, a jaw that shifts to one side, or teeth that seem badly crowded in a 5-year-old, there’s no reason to wait. The practice provides orthodontic care for children at every stage, including those early checkups.
On the other hand, if your child is 10 or 13 and has never had an orthodontic checkup, they haven’t missed their chance. Keep the conversation simple. Let your child know the orthodontist will count and look at their teeth, and maybe take pictures, and that nothing about the visit should hurt. Going to rate 5 stars since John and the back-of-house staff are excellent; kind, professional, and skilled. Unfortunately, the front desk are not quite up to par. When I called about retainer removal, I was told about a fee that wasn’t accurate/true, and when I asked for clarity, I was met with dismissive responses like “we can talk about it when you get here,” despite the fact I live over an hour away. On arrival, I wasn’t greeted and was instead cut off with “you’re checked in” before I could speak. The hostility from the receptionists was obvious and unprofessional, which overshadows the otherwise great clinical team.
