Types of Airway Treatment We Handle
Airway-focused orthodontic treatment takes several forms depending on the patient’s age, the severity of the obstruction, and the underlying skeletal cause. We assess each case individually using 3D imaging and clinical evaluation before recommending a path forward.
- Palatal expansion. A narrow upper jaw is one of the most common causes of restricted nasal airflow. Expanders widen the palate, increasing the volume of the nasal cavity and making it easier to breathe through the nose. This is particularly effective in younger patients whose growth plates haven’t fully fused, which is one reason early orthodontic screenings matter so much for children with common orthodontic problems.
- Mandibular advancement. When the lower jaw sits too far back, the tongue can fall into the airway during sleep. Functional appliances that guide the mandible forward help open the pharyngeal space and reduce obstruction. Patients with bite issues like Class II malocclusion often benefit from this approach.
- Orthodontic correction of mouth breathing habits. Chronic mouth breathing alters facial growth over time, narrowing the airway further. We work with patients to address the structural causes through braces or aligners while correcting the bite patterns that contribute to the problem. The benefits of orthodontic treatment extend well beyond straighter teeth when airway function is part of the picture.
- Myofunctional therapy coordination. Some patients need more than appliances. We coordinate with myofunctional therapists to retrain tongue posture and swallowing patterns which are factors that directly influence airway patency, particularly in children whose dental health issues developed early. These are the kinds of health concerns orthodontics can address that most people don’t associate with an orthodontist.
- Oral appliance therapy for sleep-disordered breathing. For adult patients diagnosed with mild to moderate obstructive sleep apnea, custom oral appliances can reposition the jaw and prevent airway collapse during sleep. According to the NHLBI, oral devices are an accepted alternative for patients who cannot tolerate CPAP machines.
Important Aspects of Airway Treatment
Airway obstruction often presents with symptoms people don’t associate with an orthodontic issue. Snoring, daytime fatigue, difficulty concentrating, morning headaches, and restless sleep can all point toward a structural airway problem rather than a purely medical one. In children, the signs are even easier to miss: bedwetting, hyperactivity, poor performance in school, and mouth breathing during the day are all potential indicators. Many parents assume these are behavioral issues. They’re often not.
The National Institute of Dental and Craniofacial Research has identified links between craniofacial development and breathing disorders, reinforcing the idea that early orthodontic intervention can address the root cause rather than just managing symptoms. An orthodontist with training in craniofacial biology can identify airway restrictions during routine evaluations which is something a general dentist may not screen for. The differences between an orthodontist and a dentist matter here, because orthodontists receive additional years of specialized training in skeletal and jaw development.
Timing matters significantly. The American Association of Orthodontists recommends a first evaluation by age seven. At that age, the jaw is still growing and interventions like palatal expansion can create permanent structural changes. Children who show signs of early orthodontic concerns through sleep-disordered breathing, chronic congestion, or dental crowding should be evaluated sooner rather than later. Waiting until adolescence or adulthood means the window for growth-directed treatment has partially or fully closed.
Adults have fewer options for growth-based intervention, but appliance therapy, orthodontic repositioning, and coordinated care with sleep medicine specialists can still produce significant improvement. Obstructive sleep apnea affects a substantial portion of adults in the U.S., and many cases go undiagnosed because patients never connect their symptoms to a jaw or airway issue.
