A bracket pops off the night before picture day. A wire starts poking your cheek on a Saturday. Your child takes a ball to the face at practice. Moments like these feel urgent, but most orthodontic problems are uncomfortable rather than dangerous. This guide explains how to tell the difference, what you can safely do at home, and when to call the office or seek emergency care.
When to Seek Emergency Care Right Away
Some situations go beyond a braces problem and need immediate medical or dental attention. The American Association of Orthodontists advises calling 911 or going to an emergency room or urgent dental clinic for any of the following.
- Heavy or continuous bleeding from the mouth or face
- Trouble breathing or swallowing
- A suspected broken or dislocated jaw
- A permanent tooth that is knocked out, pushed into the gums, or badly shifted
- Sudden, severe pain with facial swelling, fever, or signs of infection
Once the injury is stabilized, contact the orthodontic office so the team can check and repair any affected appliances.
Loose or Broken Brackets
A bracket that slides along the wire or has come off the tooth is one of the most common issues. If it’s still attached to the wire, the AAO suggests gently sliding it back toward the center of the tooth and using a bit of orthodontic wax to hold it comfortably. Skip hard, chewy, and sticky foods until it’s repaired, and let the office know as soon as you can. If the bracket comes off completely, save it in a small bag and bring it to your visit. Assessing the damage, protecting your cheeks with wax, and calling promptly are the core steps in handling a broken bracket.
Poking or Loose Wires
As teeth move, the end of a wire can start rubbing the cheek, lip, or tongue. Try pressing it flat against the tooth with a clean pencil eraser or cotton swab. If it won’t stay down, cover the end with orthodontic wax to create a smooth surface. Call the office before trying to clip a wire yourself, and if a small piece breaks off and can be removed safely with clean tweezers, keep it and mention it at your appointment.
Loose Bands or Appliances
Expanders, space maintainers, and other appliances are held in place with metal bands around back teeth. If one feels loose or rocks when you bite, avoid wiggling it. If it comes out entirely, put it in a container, bring it with you, and call the office promptly so the team can advise on next steps.
Lost or Damaged Aligners and Retainers
Don’t keep wearing an aligner or retainer that’s cracked, sharp, or warped, which can happen if it’s left in a hot car. Call the office for guidance. The AAO notes that your orthodontist may suggest going back to your previous aligner temporarily if it still fits, but that decision should come from the office rather than guesswork.
Soreness and Mouth Irritation
Tenderness after new braces, adjustments, or a new aligner is normal and usually fades within a few days. Soft foods, a warm salt water rinse, and orthodontic wax over rubbing spots can all help. Ask your medical provider or orthodontist whether an over-the-counter pain reliever is appropriate. After a Hit to the Mouth
Falls and sports collisions can damage both teeth and appliances. If there’s no serious injury, rinse gently, check for loose teeth or bent wires, and call the office. A dental X-ray may be needed to check the teeth, so contacting your general dentist is often a good idea as well. Keep a Small Home Kit
A few supplies make minor problems much easier to handle.
- Orthodontic wax
- A small mirror
- Clean tweezers
- Cotton swabs
- An aligner or retainer case
- A travel toothbrush and floss threaders
Having these on hand means you can offer quick help for emergencies at home, at school, or while traveling.

Going to rate 5 stars since John and the back-of-house staff are excellent; kind, professional, and skilled. Unfortunately, the front desk are not quite up to par. When I called about retainer removal, I was told about a fee that wasn’t accurate/true, and when I asked for clarity, I was met with dismissive responses like “we can talk about it when you get here,” despite the fact I live over an hour away. On arrival, I wasn’t greeted and was instead cut off with “you’re checked in” before I could speak. The hostility from the receptionists was obvious and unprofessional, which overshadows the otherwise great clinical team.
